hello doctors,Unfortunately on 17 th may night, my mother got an accident in which her right foot was crushed under bus tyre and she got fractures in 3 metatarsals and crushing of the tissues occured with a lot of bleeding.though she was taken immediately to hospital and stitches were given and temporary plaster has been done.antibiotics were also given with other medications.as i am in bangalore and she is in haryana i dont have all the information. today dressing was changed and a lot of accumulates were removed by squeezing. sir, i want your guidance about what care should be taken so that no complication occur and wound heals faster..and what should not be ignored for the best results and what are the earliest signs indicating to any severe situation..please tell me sir if you can help in this matter......

Here I want to discuss few points regarding this case which may be helpful for understanding the etiology, disease pathology & diagnosis in allopathic as well as ayurvedic point of view for the upcoming junior doctors .Before going to the discussion one thing I want to tell ….. that ….. the patient will come in contact to a physician in any stage of the disease …. i.e …… among …… sanchaya , prakopa , prasara , sthanasamsraya , vyakti & bhedha ….. that to usually the pts will come to a doctor in …….. stanasamsraya /vyakta / bhedha avastha. … If the pt will come to us as early as means i.e in stanasamsrayavastha ……. Where the pt already crossed the prasaravastha ….means already the pravriddha doshas …… spread to all the dhatus or related dhatus ……. But the pts usually comes to vyaktavastha or Bhedhavastha ….. in these situations we may not get the poorvaroopas or some times the poorvaroopas can not be recollected by the patient. So at this juncture we have to diagnose depending on what the symptoms & signs we are getting ….In the present case also the same thing is happened . Disease pathology started 5 years back and still the disease is in progressive stage … that’s why every time the presentation is different . In this case still we will get many more different clinical presentations in future. To understand this case better if we have to go through the Ayurvedic aspect as well as Allopathic aspect.Ayervedic aspect :.......

Dear friends,In our routine practice, sometimes right diagnosis of a disease itself can be a better achievement for us than giving the actual treatment. Because we tend to treat patients with provisional diagnosis and end up giving general treatment where the condition actually needs specific treatment (or strangely no treatment at all).Today I want to discuss one such case which might be useful to all of us but the treatment part may be discussed at the end of this case study as we were in a wrong track while treating this case because we took it very lightly and went on to give general treatment for a specific disease which is to the best of my knowledge has not been discussed anywhere in our literature.(Don't worry friends...our own Manish Bhai and Paresh Bhai will definitely come up with the exact reference...but as far as my knowledge is concerned, I don't know the Ayurvedic reference)I think 4 back years one male patient who was in his mid fifties came to me ( prakriti-vaata kapha, Nadi-vaata) with the c/o. Allopathic docs indulging in quackery Prescribe ayurvedic formulations in violation of apex court order Anshu Seth Tribune News Service Ludhiana, April 26 In complete violation of the Supreme Court orders, allopathic doctors are openly pres...cribing Liv 52, Softalac, isapgol, B-Vax, Glucosamine and ayurvedic pharmacopoeiac herbs, which amounts to quackery as the above formulations can only be prescribed and dispensed by a qualified ayurvedic practitioner. Interestingly, till date the fraternity of allopathic doctors had been alleging the ayurvedic practitioners of quackery for prescribing allopathic medicines but conveniently abstained from owning the responsibility of indulging in the "unethical practice" at their end. Pointing towards the complications caused to patients who were prescribed ayurvedic formulations, a noted ayurvedic doctor said: "Ayurvedic medicines are not a subject taught to the allopaths in medical colleges. Thus they prescribe them without knowing the contraindications, as a result patients end up with complications." A large number of gynaecologists are recommending ayurvedic syrups for gynae problems, which have shown adverse effects in patients as another ayurvedic practitioner referred to a formulation containing an herb "Ashok" which is given for regularisation of menstrual cycle. "But a large number of gynaecologists are prescribing the syrup to women with different problems. This has resulted in serious complications which are very difficult to reverse," observed another ayurvedic expert. Softalac has an ingredient which when consumed in absence of recommendation of the expert causes dependence. The story is the same with the anti-obesity, anti-inflammatory, anti-depressant, analgesic and various other ayurvedic drugs. ‘No one above law’ Dr LS Chawla, former chairman of the PG committee and ethical committee of Medical Council of India (MCI), who was also the vice-chancellor of the Baba Farid University of Heath Sciences (BFUHS), referred to the Supreme Court verdict of 1994 against a homeopath for causing death of a typhoid patient by administering inadequate dose of allopathic medicine. “The Supreme Court had categorically stated that a person would only be permitted to practice the system of medicine in which he/she is trained and registered," said Dr Chawla. The orders apply to allopathic doctors, homeopaths and ayurvedic practitioners. He also negated the claims of some ayurvedic practitioners that they had been allowed to practice allopathic medicine. It had also been made clear by the Central Council of India Medicine (CCIM), the body that registers ayurvedic doctors. ‘Rules should be equal for all’ Any professional prescribing medicine out of the system in which he/she is registered is a quack. Allopaths are frequently prescribing isabgol, castor oil and various other formulations for gynaecological disorders, liver malfunctions, spermatogenesis, urinary stones, geriatrics and even eye drops, which are ayurvedic in formulation. If we talk of rules and propriety, these should apply equally to allopathic and ayurvedic doctors. — Dr Ravinder Vatsyayan, guru at Rashtriya Ayurveda Vidyapeeth ‘Follow own system’ It is unjustified for a homeopath to prescribe or practice medicine outside his system and the same goes for an allopath and ayurvedic practitioner. Respected sirsMy son is suffering from Adenoids and tonsilitis i am giving him kanchanar guggul 250 mg bid and laxmi vilas ras 125 mg bid, kapha ketu ras 125 mg bid along with haridra powder.but due to severe nasal blockage he does mouth breathing and again lands up with infection. suggest me for further line of treatment.Thank you.....

Respected Doctors,2 years back,i had a patient complaining teeth ache,sensation of heat/cold and with movement in premolar teeth. at that time DENTIST ADVISE HIM FOR EXTRACTION. but patient was young(30y/m),and not willing for extraction. he came to me if ayurveda can help him. i advised him a self made dantmanjan and within somedays he was o.know after 2 years he came again for same teeth to me, now there is no pain no sensation of hot/cold, problem is only movement. he is not ready for extraction yet. i asked him for O.P.G(oral panto gram)and refer to a dentist who advise him R.C.T. (root canal therapy)followed by bone grafting.(appox.cost for both procedures is rs.10.000). he again met me to give same old dantmanjan. but dentist says if he doesnot underdone said grafting then he will lose his teeth in next 6 months or so. .....

I can give you so many examples, but today’s topic is just for my juniors who are just completed their under graduation and want to practice Shalya chikitsa.Six months back one patient came to me and said “Doctor, I frequently suffer from haemorrhoids and it causes severe pain and swelling for 2 to 3 days”I said “What happens later?”He said “The pain usually subsides”I said “Then what is your problem right now?”He said “I am OK, Right now. But Just I wanted to know that I have been operated several times for the same problem (almost 5 to 6 times in 2 years)by a neighborhood doctor (chandsi dawakhana)”He also said “I went to consult one nearby BAMS Doctor, He didn't examine me at all but explained that, it's a surgical problem” I said “If it is external haemorrhoid you shouldn't have been operated for so many times”......

A fortnight back, I received a call from one of my client present near by my residence. She was in a panic mood, while conversing and asked whether I could come & see her two children of the age group of 13 & 15 years at her residence. I obliged and went, to find those two children, presenting with a centrifugally; wide spread; synchronous; vesicular and pustular lesions covering the face,trunks,both extremities and palms. a much common complaint which a physician faces is ABDOMINAL GAS..!that is a common disease where allopathy is failed completely and ayurveda is providing 100% relief if some basic points about dosh/dushay is considered.there are a minor difference in symptoms, which decides the drug of choice from the big list of churans/vate/rasa/arks/majoons etc.(1)IF GAS MEANS BURNING SENSATION IN STOMACH REGION+SMALL CONSTIPATION,THEN DRUG OF CHOICE SHOULD BE AVIPATIKAR CHURAN WIRH OR WITHOUT SHANK BHASM............[(2)]if gas means air filled abdominal cavity,and air feeled by patient as moving one part to another causing pain+constipation.......,then drug of choice is SHIVAKSHAR CHURAN.AND IF AIR IS MOVING TO CAUSE PAIN BUT WITHOUT CONSTIPATION ,dravy mal pravartee,THEN FAVORITE DRUG IS HINGVASTAK CHURNAM WITH GRITH.beside these three basic differences,other parameters of little use are there but mostly patients got relief.as parval piste,kamdhudha may be added in thriv amlapitta(pit pradhan),kapardhak bhasm,sutshekar rasa added in case of ulcers.and so on.........
