SUDDEN FLUCTUATION IN BODY TEMPERATURE SINCE LAST ONE AND A HALF YEARS

DearColleagues and respected seniors.
last night i had a telephonic conversation with a patient. He reported sudden fluctuations on his body temperature in the mornings accompanied with headache and abdominal cramps when temperatues are high about 103 degrees.
This is non specific. All his reports have been normal.
No history of gastroenteritis or viral infections including chicken pox and measles in last 2 and half years.
NO UTI.
No history of ulcers or heartburn or duodenitits etc., reported as well.
This is relieved by paracetamol. 
The only predominant feature is temperature in morning wheneverit occurs.
I feel it is a case of jwara but i am unable to place it rightly. Kindly help me......

EMACIATION IN A 2 YEAR OLD GIRL

Today a 2 year old girl whose birth weight was 1.75 kg's and reports nromal weight gain and growth till about 7 Kg's was brought o my clinic.
She presented with -
emaciation, anorexia
she was normal in her reflexes. current weight was 7 ks's.
all her reports were normal for blood picture and stool.
The disturbing or concerning feature for me was that since last nearly 1 year she has not been passing stool well. Whatever she passes is very hard with almost no water content.
For now i have suggested - 
1 tsf of panchtikta ghrita guggulu at night
liv 52 drops in high dose of 8 drops thrice daily
kumar kalyan ras once daily with madhu.
restricted salt diet and encouraged only liquid diet.
Please suggest better alternatives to this.......

a middle aged man about 55 years suffering from chronic renal failure. his blood urea and serum creatine levels are as follows :

16 Mar 79 and 7

patient has been on dialysis twice in a week. it is a known case of IDDM and HTN.

a male boy age 15 having coplaints of recurrent vomiting and hiccups immediately aftar meal(solid and liquid both),with problem in deglutination,pain in abdomen at varios site,pain in B/L breast from last 6 month.his problem started with pain in abdomen 6 month back which laterly associated with vomiting and hiccups. OSTEO ARTHRITIS

A lady aged 60 medium built non diabetic having hypertension well under control came to me having severe back ache and after being admitted to reputed orthopedic surgeon with no relief. MRI repoted disc prolapse,nerve compression, tear, Kati vasti was offered with vish garbh tail and no internal medicine was administered initially. Patient got 90% relief. Later ashwagandha churan and triodashang guggul, R. compound BD was added.
After this cure she developed faith in ayurveda and reported her problem of osteo arthritis which is there for last 10 years or so. I continued with same internal medicine and opted for pichu with vishgarbh followed by swedan with babuna , gul tesu, punarnava, chrilla,manjith, dashmool. The crackling sounds have reduced after about 15 sittings but pain has marginally improved.
