RECURRENT PULMONARY EMBOLISM WITH HIGH COGULATION FACTOR

greetings of the day. after a long hiatus i am posting this case for discussion. 
this is about a male dr preparing for USMLE, working in a major hospital in delhi. he is 28 years old well built and apparently healthy in all biometrics except little anxiety when he talks. 
he approached me almost 2 months back for this peculiar problem.
he suffered with embolism jeopardizing his lung capacity remarkably around one year back. it started with cough, chect infection and attacking his left lower lobe ant basal and lat basal part first time.
immediately he was on medication and it resolved reasonably. second time he was traveling high altitudes wen this happened and attacked his rt lower lung. though this is under control through warferrins he is terrified if it happens again. he is suffering from high coagulation factor, in recent MRI there is no new perfusion defects. ......
