LOW BACK PAIN

Respected sir,
Plz suggest me full line of treatment and oral medicines.
Male- Age- 46, Low Back Pain 
Dosh- Vata pitta,
Kosta – normal, DM type- 2 since 1996, HTN, Lipids – Normal
Examination – SLR ++, weight- 74 kg, Height- 179 cm.
C/o Numbness on both legs, Right leg more numbness
- Muscles Weakness and muscles wasting on right leg.
- Patient feels slight stiffness in the morning
- Feeling pain when start walking after some time feels slight pain
- No pain on lumber region
- Feels pain on both buttocks 
H/o Spine surgery due to slipped disc in 1993 .......
