DYSPNEA WITH MULTIPLE SCLEROSIS

Resp Vaidyas,
Kindly help in Diagnosis and line of treatment in following case-

A male pateint of 27 years
At the age of 17, had a minor injury on one of the toe, gradually weakness of lowerlimb started, able to walk by support.. later on diagnosed as multiple sclerosis. Morden medicines taken for 3-4 years. Health was good with disability in walking independantly.
Some 4 months back started c/o difficulty in breathing and loss of appetite...diagnosed of plural effusion due to koch's. 
Tapping done and Anti tubercular Rx started.. Condition improved.

Then developed pressure sores at lower back and rt hip head area since last 40-45 days, still unhealed........
