B/L KIDNEY SUB NORMAL SIZE AND VARIOUS COMPLAINTS- VERY DIFFICULT CASE

Male pt., 51 yrs., on renal dialysis since one month, HTN, uremiic syndrome with seizures (Uremic Encephalopathy), CKD with ESRD, HTN, Secondary Albuminemia, B/L Pneumonia, Hypersensitive Vasculitis, Rt. Sub Dural Haemotoma (Acute & Chronic)with rt. Fronto-Parital infract.

ESR: 118 mm
Parathyroid Hormone: 461.5 (normal 15-68.30)
Hb%: 8.7 gm%
Protien in urine : 2+
Blood Urea: 148 (10-50)
Creatinine: 6.67 (0.50-1.2)
Uric Acid: 8.5 (3.4-7)
Low Cholesterol: 114 (200-239) 
RBC, Pus Cells, Epithilial Cells, Casts positive in urine.
Patient was asymptomatic till the age of 51 yrs with only HTN as complain.......
